REMINDERS

  1. Sign up with a valid and existing personal email address where we can send your activation link. Kindly use a personal email address (not school-related email address).
  2. Please make sure that you can access your inbox, and that the email address is correct and can receive mail. Disable any service that might block messages from reaching your inbox (e.g. boxbe, bitbounce).
  3. Enter your full name and birthdate as it appears on your Birth Certificate to avoid delays in the processing of your application.
  4. Double-check your school name. Be mindful of typos or unnecessary spaces. All information submitted in this form (except password) will be displayed in your Form.
  5. Please only sign up once for the current UPCAT year to avoid duplicate accounts.
  6. Note that upon submission of the form below, a message indicating that your account was created will be shown to you if your sign-up request is successful. Kindly sign up again if you did not get the said message.
  7. Take a look at your junk/spam folders to make sure the activation link did not end up there.

Must be your complete middle name (Please tick the checkbox below if you have no middle name)

All information in this form (except password) are considered FINAL and will be USED IN ALL TRANSACTIONS related to your UPCAT Application. Upon submission, YOU CAN NO LONGER EDIT THESE INFORMATION.
